Official Complaint Form

Send $25 by money order (to cover shipping and handling charges for mailing the documents to TIBBA's Ethics Committee Members and to the person (s) to whom the complaint is made) along with seven (7) copies of each of the following: completed form, evidentiary material, affidavits, and additional appropriate documentation to:

Be advised, a copy of the completed form, evidentiary material, affidavits, and additional appropriate documentation will be sent from TIBBA's Ethics Committee to the responding party(s).  In other words, the completed form, evidentiary materials, affidavits and additional appropriate documentation will be sent from TIBBA's Ethics Committee to the person or persons for whom the complaint is being made.  Be advised, the documents become the property of TIBBA and will not be returned.

Incomplete complaints or those without accompanied evidentiary material, affidavits or other additional appropriate documentation will not be reviewed.

Decisions made by TIBBA's Ethics Committee are not legally binding, however the decisions may be beneficial in a court of law.  If TIBBA's Ethics Committee determines that a party has violated TIBBA's Constitution or TIBBA's Code of Ethics, the offending party's membership may be suspended, revoked or denied.  TIBBA reserves all rights and remedies available should TIBBA's Executive Board decide additional action is necessary.
